Cut out two cloud shapes (google "Mario Cloud" if you want this specific shape). Sew on two felt eyes. Topstitch all the way around leaving a gap for stuffing. Stuff with fiberfill and rattle insert. Finish stitch.

I got these rattle inserts on Etsy. They come in two sizes and make a perfect rattle. You can make whatever simple shape you want. But since we love Mario and the 80s, I went with this cloud shape. 

 This toy would be nothing without the rattle. The noise is it's winning feature for sure.
